Multi Server: All servers are logged out

Description

With multi-server, a user can log out of all the servers and this brings the user to the new server screen, given the user has multiple servers he is most likely to join one that it is already there than creating a new one, so we should make it easy for the user to not have to type the whole server url/name again.

To solve this, a new ‘Select a server’ screen is needed.
